Description: Handoffs in nursing occur multiple times every day and the potential exists for miscommunication during interdepartmental patient transfers and shift-to-shift reports. Standardized handoff formats are advocated by numerous stakeholders to reduce adverse events related to communication failures to improve patient safety.This monograph reviews the nursing literature, regulatory measures and provides recommendations for standardized handoffs from the risk management and patient safety perspective.

Learning Objectives/Outcomes: Upon completion, participants should be able to:
1) Identify the barriers and benefits of effective shift-to-shift and interdepartmental
    patient handoffs.
2) Discuss the evidence-based rationale supporting a standardized patient handoff
    process.
3) Based on available tools in nursing literature, construct a standardized patient
    handoff tailored to your specific unit or patient population needs.

Learning Assessment: There will be a posttest for this activity with a minimum performance requirement of 8 out of 10 correct answers.

Estimated time to complete activity: 80 minutes

Method and Medium: To obtain CNE credit, complete a pretest, view the entire online presentation, complete the evaluation and posttest. Once completed, a certificate will be generated for printing.

Continuing Nursing Education Information: Successful completion of this course includes completion of the post-test with an 80% passage rate and completion of the course evaluation.
