 Description: Poorly documented incidents can pose significant costs to a facility during a malpractice claim. This monograph will help RNs, LPNs, and APRNs formulate easily understandable, concise incident reports. With proper documentation, Risk Managers will be able to collect the data needed to address patient safety issues and provide protection in the event of a malpractice claim. This monograph explores some of the myths and fears surrounding incident reporting, how the report should be written and steps for developing a “Just Culture” in your institution.

Learning Objectives: Upon completion, participants should be able to
1. Discuss the categories and common goal of all incident reports
2. Discuss the process of incident reporting and factors that inhibit reporting

4. Discuss creating a “Just Culture”

Learning Assessment: There will be a post test for this activity with a minimum performance requirement of 8 out of 10 correct answers.
